I agree to participate in fitness programs offered by The FitWit Group, LLC (herein referred to as FWG) including, but not limited to fitness programs known as Bridalicious Boot Camp, Boot Camp 90210, Body Blast, along with other programs offered from time to time. I recognize that exercise is not without varying degrees of risk to musculoskeletal and/or cardio-respiratory systems. I hereby certify that I know of no medical problems that would increase my risk of illness and injury as a result of participation in a fitness program designed by FWG.

I am voluntarily participating in physical exercise along with the use of the fitness/athletic equipment, training, and instruction that could cause injury to me. I understand and have been informed that there exists the possibility of adverse changes during the exercise program. I have been informed that these changes could include abnormal blood pressure, fainting, disorder of heart rhythm, stroke, and very rare instances of heart attack or even death. I agree to waive, release, hold harmless and discharge FWG and its trainers, instructors, agents, officers, principals, employees, the facility/park(s) or any person(s) involved of any and all claims, demands, actions or damages of any kind resulting from participation in Bridalicious Boot Camp, Boot Camp 90210, Body Blast or other fitness activities offered from time to time by FWG. The undersigned hereby releases FWG, as well as waives any and all claims and understands and assumes any and all risk with participation in Bridalicious Boot Camp, Boot Camp 90210, Body Blast, or any other fitness or exercise program offered by FWG.
